The Rule — Simple
During the introductory period: first treatment free on any device of the patient's choice + employee pricing on all subsequent treatments on any device. Both benefits expire at end of month 3.
Billing Notes & Operational Rules
Cash Lane — GLP-1 Included Months 1–3 Only: Semaglutide and Tirzepatide (standard) are included in the $499 introductory flat rate for months 1–3. From month 4 (baseline), drug is billed separately — not included in the $199 baseline rate. Tirzepatide carries a +$49/mo surcharge on cash lane at all dose tiers during the introductory period.
Insurance Lane — GLP-1 Surcharge ALL Months: Drug surcharge applies in BOTH the introductory (months 1–3) and baseline (month 4+) phases when insurance does not cover. Sema coach managed +$99 / provider managed +$149. Tirzepatide: microdose coach +$49 / provider +$99 · moderate coach +$149 / provider +$199 · high dose coach +$199 / provider +$249.
Cash Lane — All-Inclusive: All provider visits, standard labs, GLP-1 drug, coaching 4x/month, wellness center access. One flat monthly fee — no separate billing.
Insurance Lane: Monthly fee is program management. Covered services billed separately to insurance. Patient pays copay/deductible per plan. GLP-1 surcharge added when drug is not covered.
Intro Period — 1 Free Device Treatment: All accelerated program enrollees receive 1 free device treatment per month during months 1–3 (up to 3 free total). Any device, patient's choice each month. No distinction by device type.
Intro Period — Employee Pricing on All Devices: All other device treatments each month session are priced at employee rates during the introductory period. Applies to all devices — disposable or not. Morpheus8: $349 single / $250 add'l face / $299 add'l body. Morpheus8V: $200. Empower RF: $200. Storz: TBD. Bemer/Red Light/Axon/Apex: TBD. Both benefits expire end of month 3.
Rediscover Him/Her: $199/mo cash / $99/mo insurance flat — labs and all provider visits included in cash lane. 6-month minimum commitment. Year 1 prepay 20% off. Year 2 loyalty 20% auto. Year 2 prepay 25% off.
Transitions: Accelerated programs auto-transition to baseline at month 4 — month-to-month thereafter. Rediscover requires 6-month minimum before month-to-month.

Member Benefits — 6+ Consecutive Months on Baseline: Device procedure discounts, promotional coupons, digital punch card (first maintenance treatment free per device category). Consecutive months required.
