Insurance Lane — Initial Provider Visit Required: Insurance patients must have an initial provider visit to establish the IV treatment plan before IV therapy can be billed. ABN required for all therapeutic infusions — likely non-covered.
Cash / Self Pay — Provider Visit Included: Cash pricing includes the provider visit. No separate billing for the provider encounter. One flat price per session.
Virus & Immune Category — Higher Pricing: Immune Booster and Virus Buster are $229 cash / $169 insurance due to higher ingredient cost (Vit C 5–10g, NAC, L-Lysine, GSH, Zinc). All other formulas are $189 cash / $129 insurance.

Billing Codes — Therapeutic Infusion: 96365 initial therapeutic infusion (≥16 min) · 96366 each additional hour. ABN required — commercial payers unlikely to cover wellness IVs. Medicare/Medicaid: ABN required.
Billing Codes — Hydration: 96360 initial IV hydration (31–60 min) · 96361 each additional hour · J7030 Normal saline. Use hydration codes ONLY when documented dehydration is present. Do not bill hydration for vitamin infusions unless true dehydration is documented.
